You just answered your own question. Your cold idle prolly isn't tuned properly. And if you're running rich, your ECU is definitely not tuned properly. Your engine is probably in horrible running condition... So, give your car a tune up and take your car to a local tuner and have them properly tune your car.
I'm thinking it could be the following:
