Crank Pulley Problem

Crank Pulley Problem
OK..I have a couple things wrong with my crank pulley and belts. I had a key way break on me a couple years ago and I ruined an altenator belt and pulley. I replaced the key, belt and pulley and a year later the key way let loose again and had to replace the key and crank pulley. I put an OEM pulley on and key way. I tighted the bolt to about 100 ft-lbs torgue and put lock tight on the bolt. When I start the car the pully doesn't spin in a circle more like an egg shapped rotation.
Secondly is how do I know when my belt are too tight? I have my power steering belt tight but still have hard spots when I turn my wheel all the way one direction and turn it back..Does this mean I may need a new power steering pump?
I have had my honda mechanic look at it and doesn't know why I keep have this problem of the bolt coming loose. All my parts are OEM Honda parts from the Honda dealership. Also when the car is on inlcine and I start the car the belts squeak but when I am a flat surface it doesn't squeak at all. I have no idea what is wrong with my car but it is driveable but scared that the pulley will loosen again. If any one could help I would greatly appreciate it.

I meant to add its a 96 civic ex five speed. Could it also by the the motor mounts need to be replaced and cause the belts to loosen and tighten and cause the pulley to get caught or yanked on my the belts?
