First off, what's the method on your octane? It should say right on the pump.

Second off, when was your last full tune-up? Plugs, wires, cap, rotor, air/oil filters, oil change?

Third off, start driving your car like a grandma. If you're revving past 3k, you're doing gas mileage wrong.

That's around 18-23mpg....
Using higher octane isn't necessary, am I wrong?
A Honda Civic isn't a race/exotic/luxury/sports car, it was meant to be a typically reliable commuter of its time, hence the root word Civil/Civilian/etc.

Any CEL's? Any problems at all? You didn't answer the question regarding a tune-up.
Ignition: Distributor cap, rotor, plugs, wires.
Fuel: Filter, maybe even pump
Air filter, oil etc;
Engine ignition/mechanical timing in check? Intake manifold having any vacuum leaks?
Are you constantly blasting the A/C?

Exhaust system fine? No O2 sensors bad, or a P0420 code?

Time for a tune up, usual suspects.
Plugs
rotor
air filter
etc...

Keep in mind it has 190K
She aint what she used to be.
Id also ditch the 91 octane gas

^^^THIS...BOOM! haha

Also, poor wheel alignment can loose you MPG. You must also realize you have and AT. Although you should be getter better MPG, you are loosing a lot of efficiency in that old transmission.

i have no CEL, i checked the o2 sensors and they read good as new, i do regular oil changes,i do change my fuel filter from time to time, but usually every 6 months. how often should i change my spark plugs wires, i never changed those, and never changed my rotor and cap as well. it used to do better while using ac in the summer, all my trips are inside the city as well.

at 190k, you should replace them now. They are technically "lifetime" parts, but for optimal running, they should still be replaced. I personally replace them according to the FSM for plugs, which is every 30k/24mo.
