Default jrsc b16 problem

my friend just had his jrsc installed on his 99civic vti (the same as your si)
when they installed it they had some problems with the fuel line i think, but they thought they had gotten that figured out?
the car idles at 1500rpm.
he had it dynoed and everything was running smoothly.
then he went to the track and did some runs, then he felt the car chocked sometimes when running in 4th gear.
then when he was driving home this afternoon, he felt like the car was gonna die and stalled, and he decided to drive to the guy who installed it and check if he could figure something out, then the car again started to chocke then he looks in the rearview mirror and sees the car is blowing blue smoke and the car then died, then he tried to start it up and it seems to run on only 3cylinders.

do you have any ideas of what could be the problem?
