I would recommend getting a OBD scanner, then disconecting your injectors while the car is running one at a time. Don't disconnect one, then disconnect another. Just check each. Not sure it that makes sense. This should give you an idication of which one is bad if that's the case. The OBD scanner can probably tell you as well without having to unplug the clips from the injectors...

Take a long screwdriver. Put the blade end against the injector body and then press your ear against the handle end. If the injector is working, you'll hear a "tick.......tick......tick......tick....." as it sprays. If you don't, that one's not spraying.
I just had my injectors cleaned and rebuilt by RC Engineering. They came back looking like new, working like new and with a full "dyno" plot of their performance before and after service. They did all 4 for under $100 total. When you find the culprit, give them a call!

Code 16 is set when there is a break in the electrical path for the injectors. This could be caused by a poor connection, bad wire, injector coil about to go, or even a brief main relay failure.
