have you had the tranny apart for any reason? Alot of time people that have no experience taking tranny's apart try to install lsd's or gear-sets and when assembling the tranny the shift forks get messed up. I had to pull apart a tranny for a guy that had really similar problems to yours. After getting the tranny apart the forks were all messed up. it was an easy fix but I'd suggest taking it to a reputable tranny builder.

With the clutch pushed in, try to put it into 3rd first and then into first. Third gear is what lines all the gears up. This might help.

I had the same problem when I was using heavy weight tranny fluid. The problem went away when I replaced it with Honda MTF.

I had the same problem on my d series. I dropped the tranny and put in a new throw out bearing and it works perfectly now
