Default 96-00 Skunk2/OEM Shifter Question

I recently started getting a loose/rattle sound coming from the bottom of my shifter. Last week I checked under the car and the main bolt holding the shifter to the shift rod was a little loose.

I tightened it up snug but then it made my shifter impossible to shift, like the bearings on the bottom fo the shifter were stuck. So I loosened it up enough to shift fine with no ratlte and it felt great for a week and now it's vibrating again.

I haven't gone under the car again but all I remember seeing outside the skunk2 shifter was two rubber washers that sit outside the shifter bearings.

I'm looking at this:

And it looks like there's supposed to be some metal collar/sleeve that keeps the shifter bearings from binding when you tighten the bolt down.

Again, all I have is the skunk2 shifter with the pressed bearing inside the bottom and the two rubber o-rings. Is there supposed to be anything else down there to allow the shifter to be tightened all the way without binding?

I tried looking for pics but couldn't see anything clearly.
