Default Help diagnose problem - LOUD noise at certain rpm's ('99 civic)

Tranny (ex/si) has about 110k on it, clutch (exedy organic) and flywheel (oem replacement) have about 30k on them. The tranny has had a slight grind going to 2nd since I put it in 10k ago, hasn't worsened much. The clutch does not slip. As for the noise, it sounds like a muffled gear grind, but is quite loud and obnoxious. It comes at certain rpm's, not sure exactly when, but probably around 4,000. I feel like it's too early for it to be the clutch or flywheel, so it's likely something in the transmission. Anyone got an idea?
