Corrected information in ()
wiring harness (use your original harness, extend plugs. Because the civic's after 96 have a one piece harness)

crv alternator +2 mounting bolts (any 96-01 b-series integra or crv alternator)
94-00 ls starter + 2 mounting bolts(any b-series starter from 94-01 integra)
96-00 LS intake manifold- loaded (fuel rail, injectors, fpr, and evap purge solenoid)
96-00 LS throttle cable (94-01 integra ls throttle cable)
96-00 LS intake manifold gasket
96-00 EVAP purge solenoid
96-00 IAT sensor
B series header (99-00 si, 94-00 teg)
96-00 LS exhaust manifold gasket (94-01 ls the same)

MOUNTS:
99-00 si rear mount bracket (94-01 b-series rear bracket ls/gs/gsr/typeR same or 99-00 si rear bracket) AKA: T-bracket

94-00 LS/GSR/Type R front trans mount bracket with bolts
96-00 CRV driver’s side front lower engine mount bracket (only if you want to maintain A/C, otherwise, any 94-01 integra or 99-00 si bracket will work)

99-00 si timing belt bracket (keep the bracket that comes with the engine, will bolt directly to your factory mount)

ECU:
96-99 OBDII LS ecu p75 (00-01 integra ls ecus have immbolizers)

Transmission:
LS Hydraulic (any b-series hydro transmission bolt will work)
LS trans mounting bolts (any b-series, shares the same bolts)
clutch fork
clutch fork pivot bolt
clutch fork release spring

Clutch:
99-00 si or LS/GSR/Type R (any dohc b-series clutch from 94-01 integra, 93-97 dohc vtec del-sol, and 99-00 si)

Any b series flywheel

94-00 Integra NON ABS axles/intermediate shaft (ABS axles works fine)
94-00 LS/GSR/Type R shift linkage + bolts/washers/bushings

missed parts and tips
-99-00 si intake box to pass bar
-exhaust system will be modified in order to for the header to bolt in place.

Definilty good information, my 2 cents.

If your going from D series to B series its harder/hassle/expensive to jump into a b20 setup.

Try and find a full b18b setup swap that in your car a year down the road when you get bored swap out the b18 and swap in the b20 and get a chipped ecu .
