Default One high beam won't go on?

My cousin has a 2000 Civic sedan, and her driver side high beam does not come on. IT IS NOT THE BULB AS I CHECKED THREE DIFFERENT H4 BULBS. The passenger side works perfectly. I also believe the high beam dash light does not come on either.

Anyone have any ideas what could cause this?

I figure it can't be fuse, since the low-beams do come on perfectly and only one high-beam works... if the fuse were bad, none would work.

Do the headlights have separate relays for the high beam? One for each headlight?

The high and low beams share the same ground. Check whether fuse 4 or 5 is blown. It's probably fuse 5 as it also controls the dash light for the high beams.
