How do you know when rocker arms are bad?

Im refreshing my obd0 cylinder head and the rocker arms show some wear but im not sure how much wear is too much... when do you know its time to replace the rocker arms? Or do the pretty much never need replacing?
I have never seen the need to replace the factory rocker arms... NOW, the lost motion assemblies do cause a lot of valve train noise. That is probably the issue, if you are hearing valve tick (as long as your valves are PROPERLY adjusted). I used to re-build them w/ moderate success. But it is too much of a pain-in-the-***, to do it twice. SO I usually recommend buying new ones.. and they are not cheap.
I have also used the Crower VTEC-delete roller-rocker assemblies, w/ minimal success. They are a bitch to adjust valves and the roller bearings started to fall apart.
There are othe companies offering a polished and/or lightweight factory-type rocker assemblies, but they are expensive.
