What are the causes of loss oil?

What I know so far.
1. Broken oil pan
2. Blown headgasket.
What else? Also, include an explanation of why it does it mechanically

Bad/worn gaskets/seals
Clogged PCV system (excess crankcase pressure will force oil past gaskets/seals)

Which is caused by worn rings, or leaky valve stem seals.
