Have you or anyone messed with the timing belt at all?
I'm thinking one of 2 things is wrong.
1) The timing belt isn't tensioned properly because either the tensioner pulley is bad or the belt wasn't tensioned properly when it was installed or played with.
2) One or both of the two alignment plates that sandwich the crank gear might be missing.
