Use the b-series shift linkage and pop the civic shifter (2 10mm and a pair of grips) onto the gsr/type-r linkage. Either b-series linkage should work, I'm pretty sure they are identical. Also on the stabilizer bar, there is a counter weight at the end (right next to the shifter on the shift linkage, cut it off or you will possibly get metal-on-metal grinding noise in your shifting. A Hack saw will do it or a torch if you have one. Should take no longer then 5 minutes tops. I do not have any pics, sorry. The counter weight is obvious though.

You might be able to use the GSR shifter, I've never tried, as I've always wanted my stock civic shifter feel. Plus the man recommended it changed, so I did.

You CANNOT use a D-series (SOHC) linkage with a B-series swap, totally incompatible.

You need to use a B-series shift linkage from one of the vehicles listed below and it should come with a shift lever when you get one. You shouldn't have to swap your SOHC shift lever over like the fella above mentioned:

- 94-01 integra (any 5-speed model)
- 99-00 civic Si
