Screw in upper right hand corner, screw in arm rest (trench), screw behind door handle, clips all around. Pop the clips then lift the door up over the window sill before tugging. Also, you need to get the window crank off.

If you need pics I believe there's a link involving replacing the stock mirrors... and in it he goes into details on how to remove the door panels.
The instructions above pretty much explain everything though.

Re: how do you remove a 98 civic door panel (Judd)
-Door handle - 1 screw
-Remove lower cover of arm rest - 2 screws
-Remove sail panel
-Remove speaker grill cover - 3 screws
-If you have power windows, roll them all the way down first.
Starting at the bottom of the door, pull out firmly, there are 6-8 clips holding the door on. Once you get it loose, unclip (if you have power options) the harnesses for the power windows and door locks and pop them back through the door.
Lift straight up on the panel and pop it off the lip from the window. Turn the door panel at about a 45 degree angle, with the front edge of it being lower and the rear end of it up in the air kind of in the window area and work the door handle through the panel. It helps if you pop the handle bar out of the first clip closest to the handle to give yourself more room to move the door panel. Get the door handle free and the door panel is free and clear, just pull it away now.
