Usually gently tapping it from side to side with a hammer will loosen it up after you remove the bolts.

You have to remove all the cam caps with the rockers as a complete assembly. After it is off of the head, you can disassemble it. Hold the cam cap rocker assembly by the ends and be careful not to let it fall apart, because there are a lot of parts.

It lifts straight up. Some of the cam caps have dowel pins which may be stuck. Detach those by prying straight up. The camshaft and the oil seal will stay with the head.

The cam caps are machined with the head by a machine that bores the cam bearing holes through both the cam caps and the head. It's a matched set. Cam caps are not sold separately. You will need to repair the one you have or replace the whole head or consult with a machine shop for other possible options.
