Get a Helms.
Get a meter.
Measure the O2 heater resistance (as the Helms manual instructs one to do).
Spec is 15-40 ohms.
If it's out of spec, replace it. If it's in spec, follow the Helms troubleshooting outline for futher investigation. There aren't but 3 parts to this 'system': The O2 sensor itself, the wiring harness and the ECU. It's gonna get spendy replacing the other two vs troubleshooting correctly - your call.

I don't know. I generally fix what is broke on my car. I've found that leaving a problem go un-fixed only causes bigger problems down the road.
