Default 99 civic code P0131

Hey guys, I'm throwin a damn CEL again! well I just finished up the swap to a B16A2 SI motor and last week I was throwing code P0135 some o2 wiring malfunction... Well I got that all fixed up and the next day I threw code P0131 and it says something like heat malfunction code... I have been re searching allover online and everyone has said its a very difficult code to get rid of... Have any of you guys had they same problem or know anything about this code?? Sorry if the post is hard to read I am on my phone thanks for any help tho guys.

and i also forgot to mention that I have an exhaust leak from where the headers meet the cat...

mine thru the same code on me after i did my b18c swap in my 99 and yes the leak between the header and the cat will make the code come up but make sure also you check the o2 sensor clip to make sure its not loose and make sure the o2 sensor is tight and it should go away i havent had mine pop back up for over a year and still going
