Default 2nd's kicking out of gear, plus noise?

Today started out normally for a vehicle with a deteriorating clutch. But after driving 45. I began to notice a weird kicking throughout 2nd, 3rd, and 4th. Also, sometimes while accelerating in 1st, there is a loud, whining noise, similar to a noise I hear while in reverse.
Here's a video with the sound, sorry it's not the best.
Please post if the video is not working.

Sadly I think it did run dry... I've never really been able to know how much fluid its in it.. But I feel like the output shaft seals weren't properly installed out possibly were even the wrong ones?(are all d series axles/transmissions the same size?) Is anyone selling a d series tranny with low miles in wisconsin?
