Clutch problems

Clutch problems
First, the clutch pedal had to be fully depressed in order to shift it into gear and it grinds everytime I move it. Sometimes though, after driving it for a while and had been shifting quite a bit, it just wont engage regardless whether the pedal is fully depressed. I thought that I only have a leak since the clutch pedal wont fully go back up.
The clutch was replaced at the beginning of the year but I had the flywheel resurfaced only. Also, the master cylinder was replaced at the same time.
Oh, before I forget, whenever I am on a stand still and would move a little bit while I press the clutch lightly while the other foot is on the gas, the front end would shake like crazy.
Any thoughts on what could be wrong? I need to get this one fixed since its my DD.
