Default odd clutch engagement issue

Kinda hard to describe but here it is: Lets say you are driving, the moment you put any pressure on the clutch pedal, the clutch starts to disengage and starts to slip. I've tried adj my pedal and all that does is increase the amount of slack on the pedal before it starts to put pressure on the master cylinder. So its like having a very VERY high clutch engagement.
Am I making any sense? Is my clutch wearing out? I have a exedy stg 2. b16 tranny

Here is my setup https://honda-tech.com/zerothread?id=2035882
