Default Injectors bad??????????

(keep in mind i had these injectors rebuilt and flow tested) alright guys im stumped.... if anyone can help. setup: built ls stock cr t/4 dsm blue tops, ok the tune i had was pretty on to begin with. would run pretty decent until about 5k then load up down to around 10.3..(i know thats shity but beside the point) ok so the other night i just took off hard in my car from first and when i pulled into second the car shot pig rich and has been stuck that way every sense... any ideas? even at idle its lopey idle and staying around 10.0 on the wideband... any amount of throttle takes it up to arond 12.0 then goes right back to 10.0 when i get to part throttle again.

what chip and what chip reader?

most likely you just have to set the offsets correctly and be reading the appropriate chip in your burner software
