The block guard is a piece of aluminum that is press fit into the water jackets around the free standing cylinders. the guard helps to stabilize the cylinders under pressure. Sleeves are generally ductile iron, and replace the stock sleeves in the car. Depending on what engine you have, if you want to run forged pistons, you will need to have the sleeves done. In my Prelude the stock cylinders had nikisil lining on them. You can't run forged pistons with nikisil lining. Cost wise Sleeving the block is more expensive. And most people that I have talked to don't like the block guard anyway. Sometimes you can run into overheating problems with the blockguard.
