Default HELP OVERHEATING PROBLEM

searched the forums but well i tried the things it said like the thermostat and cap, but its still overheating.
like when the car gets warm, eventually after about 10 minutes of freeway driving, when i have to go up some small hills, the things starts losing power and then overheating. Then gets really hot and then boom steam cus the coolant is shooting out of the resevoir line. empties most of radiator fluid. wondering what i should replace? fan blades are slightly cut due to the turbo and i have a b16 with the regular 92 civic radiator, but yeah im not sure if thats it??? thanks
