Use the top one for Vac. and the bottom for Boost(only if you want to its not needed)

You can leave it open.
Connecting it will help with compressor surge at low boost, it's meant to be connected to the compressor outlet. I have it unplugged for now, and I get a bit of compressor surge no matter how much I loosen the spring tension.

Wow, I'm confusd. . . Does this make sense? It seems like you're saying to hook the boost tube to the bottom side of the diaphragm, which will essentially fight against the spring pressure when you're under boost, and want the BOV to stay closed. . . Did I misunderstand?

The pressure will be the same or equal at the top. It will make the BOV more sensitive when there is a little bit of boost at the compressor outlet but very little or none in the intake manifold...
