Usually that's kind of a catch-all code for fuel-injection system problems when the ECU can't actually point to a particular reason. For example maybe fuel pressure is wrong. Maybe an injector isn't spraying properly, even though the electrical circuit of the injector is OK. Maybe your O2 sensor is flaky, but not bad enough for the ECU to recognize it's bad.

Got an exhaust leak near the O2 sensor? That can make it read wrong without the sensor actually being bad.
How old is the O2 sensor. They don't last forever.
Tune-up? How old are the spark plugs, cap, rotor, wires? How long since valve adjustment?

How hard to do valve adjustment? they do sound when I step on gas sounds like automatic old honda .. Also I got aftermarker exhaust and there is a hole and I see water coming out in MID pipe is that normal? o2 could be bad? did repplaced plugs and stuff at 75k Thanks.
A valve adjustment is pretty easy for me, but hard to guess for you. What's the biggest car-repair job you've ever done? There's a write-up here or maybe on team-integra or somewhere.
Leaking exhaust usually gets blown out behind the car as you drive. When you sit in traffic maybe the exhaust collects around the outside of the O2 sensor? Hard to guess whether that's enough to set this off, but it's suspicious. This doesn't damage the sensor, it just screws up the measurement. (As long as you got a leak, the water is a normal part of the exhaust gases.)
New plugs at 75k miles? What does that mean? If you got 130k miles on it now you need new ones again.
