Default competition clutch stage 4 problems

hi ... recently at the track, the car wouldnt get into any gears when the engine was running ... after taking the transmission off to inspect the clutch, i discovered the competition stage 4 clutch i had was in real bad shape

- the competition clutch stage 4 that i had was sprung ... ive put about 5,000 miles on it on the street ... it was babied for the most part, and its been to the track about 3 times ( probably 15 runs total ) with 22inch slicks launching at 7,000 rpm .... the motor puts down about 240whp and 150tq

- my questions is .. is this the usual life of a clutch like this ... or did something else cause the problem ? .. maybe installed improperly, bad quality ,etc ... any advice or suggestions ? ... here are some pics of the clutch .
