wierd IACV problem

Default wierd IACV problem

ok im having a very hard time getting my car to idle correctly i have a 1993 accord coupe with a Dohc F20b SIR-T version the ecu is a chipped P28. First of all i Tried using the obd2 iacv but got it to work only once then never again so i called up a friend of mine an borrowed his H23a1 plenum so i could use an obd1 iacv and found out the iacv that was on it was bad so i ordered a new one and installed it and fired up the car, at first it idled smooth at 1500 rpm then the idle slowly rose to 2k and dropped down to 1300 and started surging but it didnt throw a code. So i turn the car off and turn it back on and it revs to 2k and dies and throws the iacv code 14 ....wtf, so i tried out the iacv on a friend of mines accord and it ran flawlessly so a few days later i went to his house and hooked up my WB and did a little bit of street tuning and found out it was running hella lean at idle between 28:1-35:1 a/f so we tried richening it up a ton but it barley changed at all, but we got it to run between 14.5-14.8 at part throttle and around 13 at WOT out of vtec. Another thing when i rev the car to between 2500 and 3000 rpm itll idle smoothly at around 800-1k rpm.

Does anyone have any ideas as to what could be causeing the iacv to act like this cause i want to get it idling normally before i get her dyno tuned?
