It's a camshaft with different cam lobes that engage at higher rpms. Depending on which engine you're talking about, it does different things. The ones in a B18C (Integra GSR) give the engine good breathing at high rpms, but go back to more normal cam profile so it still behaves normal at lower rpms. On other engines Honda sets it up to improve emmissions or improve gas mileage, or whatever...

The way vtec works relies mostly on your cam lobes. On DOHC engines like the H22, there are two sets of lobes on each shaft. The smaller lobe is the non vtec lobe, and the larger lobe is you're mac daddy vtec lobe. When the engine reaches a certain RPM rate, the ecu (basically) kicks the piston to the larger, longer vtec lobe, allowing for some extra oomph. The reason this creates more power is because the longer lobe is more efficient at higher engine rpms than the shorter one is. It's the same reason why stage 2 cams at idle sound rough, becuase they are less efficient at lower rpm ranges. V-tec was hondas answer to trying to mix both ends of the spectrum, high end power vs. low end fuel efficiency. The only bad thing about vtec is that people constantly misspell it, which inevitably ends in someone getting smacked with a valve cover.
