timing belt about to fall off??? any reasons why?
i had my head switched about 30,000 miles after my timing belt was replaced and it was loose as **** and floppin around hittin the cover. push it back onto the camgear and make sure the tensioner is tight. if it happens again id look into the tensioner more
slide it back on, put the motor at #1 cyl TDC and make sure the cam timing is correct.
Then loosen the tensioner bolt and rotate the crank 3 teeth past TDC counter clockwise to properly tension the belt, then tighten the tensioner bolt agian.
taken my timing belt off so many times i have memorized the service manual

also, if you have the upper cover off you should be able to look down and see the mark on the oil pump housing and just watch as 3 teeth go by and tighten the tensioner bolt back up.

Is that how you found it when you removed the valve cover?
Could be missing a thrust washer down at the timing belt drive sprocket.

Yeah, sounds like you are missing #12.

I did this once, but I had the singular pleasure of watching the belt actually start to work its way off before I yelled for my buddy to shut the car off. It was one of those "slow motion" experiences.
