Default Shifting problem.... under braking?

My girlfriend's 94 Integra LS has had some transmission woes. At first it made a horrible noise during clutch engagement and then it became diffucult to shift. Turns out that a spring had popped out of the disc hub, so we got a new oem clutch. That solved, it is still having trouble downshifting into third and second, but only every once in awhile AAAAAaaarrrgghh!!! After testing it during different conditions, it seems to have trouble downshifting under braking. What?

Most of the time the tranny works just fine. Only when the brake pedal was depressed significantly would the shifter "gate" be closed. Any help or hypothesis is welcomed, my crude understanding of trannies is pretty much at a brick wall.
