Default Rebuild a 4cyl as a 3cyl?

I noticed that the geo metro has a 3 and 4 cyl version of its car, but was wondering how you adjust the cylinder's degrees of separation. Like 6 cylinder engines are more smooth because there is always a cylinder firing every 60 degrees. I was curious about the idea of taking a D15 series engine and simply removing the piston and rod, valves, etc. for one of the cylinders. In order to make the engine not run unbalanced, you'd need to adjust the other cylinders so that they come up every 120 degrees and change the valve timing, right? Is the crankshaft like the camshaft in that it's premilled to a specification that allows the pistons to come up and down after a given time? If so, would this mean you'd need a new crankshaft as well? Or is it possible to simply remove the right piston, therefore making each piston come up every 120 degrees?
