O2 sensor problems, code 41, help..

Default O2 sensor problems, code 41, help..

I recently swapped a d15b vtec into my 92 cx, and since I'm using a stock p28 ecu, I realize I have to use a 4 wire o2 in order to stop showing code 41. My car originally came with a 1 wire o2, so I got a 4 wire from a gsr, along with the o2 sensor harness and the 4 wire plug from an ex engine harness. I have the orange / black wire going to the top center pin on the shock tower harness (orange / black wire, I tested continuity between it and pin a6 on the ecu). The yellow / black wire goes to a switched 12v (tapped into another yellow / black inside the engine harness), the white wire goes to the original sensor wire from my 1 wire, and the green / white wire is tapped into another green / white wire inside the engine harness that goes to a sensor on the thermostat housing, which tests as a ground. However, I reset the ecu and I'm still showing code 41 ?!?! Please help, thanks..

Edit: I tryed plugging in a buddies primary o2 sensor from his 98 civic lx, and it still had the code.

you got it wired wrong.

there's only 16 possibilities. use a voltmeter and test where each wire goes.

do a search on the exact pins that the wires should go to as well.

I gave a brief description of where each wire goes in my previous post, and I did test these wires to make sure they are where they're supposed to be, according to an article on hybrid-perf or something (I think?). Anyways it says that the orange / blk should go to pin a6 on the ecu, the green / wht should go to the common ground wire that is shared between the iacv, ect sensors, the white wire should be the same signal wire as the one from the 1 wire, and the yellow / blk wire should be a switched 12v source, as the injectors use, or the one sensor on the back of the intake manifold, etc etc.. I checked all my connections more than once and tested to make sure they're all in the right place according to the directions I was given, and all connections are soldered and taped inside the factory engine harness loom. What a pain. Maybe you can tell me where each wire is supposed to go? Thanks..
