New Clutch
I just had a new clutch an pressure plate put in my 97 Accord. The clutch and pp have about 3000 miles on them.
When the I accelrate hard there is a noticable skake/shimmy through 3500 to 4500 rpm. Its not real bad and I do not think that the clutch is slipping but I would not rule that out. It can be felt though the steering wheel a little and through the accelerator pedal a little and through the floor a little.
The Haynes manual describes a similar problem and atrributes it to CV joints. Does anyone have any ideas?

Your problem seems to be either related to your CV axles or your clutch/flywheel friction surface.
Do you have a full-faced disc or puck-type friction disc? Is it sprung?
Was the flywheel PROPERLY resurfaced before the clutch was installed? i.e. stepped correctly.
Were the pressure plate bolts properly torqued to the flywheel?
Was a new release bearing installed?

All components were new. The flywheel was probably not resurfaced as it was new. I assume the mechanic torqued all bolts to spec. I am getting ready to call him as it has been well over 3000 miles and I still get the "rumbling" vibration around 3000-5000 rpm.

It seems to me like your first guess was probably correct. Hard to say without driving the car though.
Ask your mechanic if he remembered to thoroughly clean the friction surface of the new flywheel before installing it. Many times the anti-rust coating the manufacturer applies is not completely removed and the remaining coating creates hot spots and therefore a warped flywheel. (think brake rotor shimmy).
