Fuel Injectors Code 16....Help!!

Fuel Injectors Code 16....Help!!
I just changed my fuel rail to a stock GS-R one on a B16a. After that everything was fine until the next day im in 3rd gear at 7k and i get a Code 16 and my ecu goes to Limp mode, like i hit my rev limiter. Wiring has been the same for years so no problem there. Could it just be i need new injectors or a main relay?? Has anyone ever exprienced this, please help!!
And if it matters the car starts right up everytime and is able to go to 8k and others it cuts me off with the code 16. Thank you in advance for any help!!
