Default Crankshaft Pulley always gets loose

I have a 4 cylinder 1994 Honda Accord EX and my dad used it too run an errand 4 months ago. on his way back he heard a weird noise and the crankshaft pulley had fallen off. we replaced it and it gave me no problem for about a month and it fell of again. We got three mechanics to look at it and two said we had to replace the crankshaft. the third mechanic said we didn't have to replace the crankshaft and he said it was just the wedge of the pulley was worn out and the woodruff key was worn out too, so he went and replaced the key and the pulley for us. The next day after that I went to turn on my car and the pulley started to get loose again. Also one of the other mechanics is telling us just to weld the bolt into the crankshaft and to later just replace the whole crankshaft. Also I am not the first owner of the car.

I was wondering if anyone would know what else could be the problem, or if I am going to have to weld it or replace the crankshaft.

The splined teeth are probably worn to nothing and its not staying and spinning loose.

You "could" tack weld it if your in a pinch or just don't care, but the proper fix is to replace.

what are you torquing it to?

and yes it does sound like there may be damaged to the snout of the crank, with the threading in the crank as well as woodruff key and slot.
