Code 1!!!!!!!!!!

Code 1!!!!!!!!!!
I'm running a 95 sohc vtec block w/ 97 sohc vtec head. Ever sence the head swap I have been getting engine code 1 02 sensor. I replaced the 02 sensor with a brand new one and I still get the code, immediately after the car is started. I know I have a vacume leak at my throtle body temperarily, could this trigger oxygen content engine code 1?

MIL Code 1:
Primary Heated Oxygen Circuit
High/Low Signal
Prolems relates to MIL code 1:
Open or short in Primary HO2S sensor
Primary HO2S sensor
ECM
So it isn't just the O2 sensor...

this happens alot when people forget to unplug the o2 sensor from the harness when taking off header or pulling out motor ... so the wires get rippe fromt he plugs, they put the pins back into the plug but the two black wires are still switched around ...

Nothing has been pulled at all. I replaced the old one with a brand new 02 sensor and it still throws the engine code 1. I have no clew what it could be at this point.
