Default Clutch disc failures?

i did a clutch job on a friends G20. he got the clutch kit off ebay, some japanese brand, not diaken/exedy tho. one month and only 500 miles later, the transmission just stopped functioning and a lot of rattling noise inside. actually thought it was an axle that popped out, but it wasnt and we just took out the transmission and found the clutch disc in shambles. it seems the rivets and sheet metal that connect the outer friction material and the inner spline just totally came apart and ripped up the sheet metal connecting the two parts. is this a common failure? we couldnt think of anything other than quality defect that could cause this. as far as we know, we installed everything properly, everything else was ok.

actually, comparing the kit my friend bought which claims to be the exact same as OEM and reboxed, its no wonder it failed in the way it did. im talking about the metal tabs that connect the inner portion to the outer friction ring. take a look.

whereas, this is the typical exedy kit.

im wondering if the OEM one we replaced was in fact the same as the first one pictured that we replaced it with. i would think i would have noticed the difference. hmmm... oh well.
