Default h22 civic ek vacuum leak?? Help!

I have a 98 ek ex with a h2b swap. I have a problem driving the car because of a vacuum leak (I think). Anyway. When I tap the gas pedal quickly it sounds like the car is bogging out and rpm drops a little. If I gradually(slowly) press the gas pedal down it revs fine. BUTTTT, if I hold the gas down above 2k rpms then the idle will drop down and go back up like a vacuum leak. Only if I hold the gas pedal down. When I drive the car I can't go past 2-3k or the car will start jerking really bad. Any help please!!!? Thanks.
