98 GSR ECU code 61 thrown: Primary O2 Sensor

Default 98 GSR ECU code 61 thrown: Primary O2 Sensor

I found out that my CEL light is on b/c code 61 is being thrown. It's the Primary Oxygen Sensor. What could be causing it? A bad o2 sensor, or could the o2 sensor not be put back in when I had my header installed?

It's an actual malfunction with the O2 sensor. If it wasn't connected properly, it would be throwing a code 65.

Correction it wouldn't be code 65 it would be code 41. Got 1st and 2nd O2 sensors mixed up.
