Default B16 stays in Cold Start mode even after warmed up

Hello HT- looking for some possible solutions on this issue that I’ve had for a few years now.

I have a EM1 B16a2 with basic bolt-ons. Stock P2T ECU

Few years ago I installed a Skunk2 Ultra Street IM w/ a 72mm OmniPower TB (I believe this is the size or close to it).

I transferred all components from stock IM & TB to the new IM and TB (IACV, sensors, fuel components, etc.)

The car fires up just fine and if cold, begins cold start cycle and idles at 2,000 RPM. For some reason, after the car warms up - the RPM remains at 2K and doesn’t go down. I tried giving it gas, driving off like this, cleaning and swapping IACV and nothing seems to help.
The only fix I found is restarting the car by turning it OFF & ON when it’s warmed up — this seems to work but it’s an annoying process.

Any ideas what it could be? IACV, TB, Tune, fuel???

Put your stock throttle body back on and see what happens.
