Is this o2 sensor bad? (pic included)
Can't really look at an O2 tell if it is bad or lazy.
Would need to monitor voltage output to see if the sensor is faulty.
If the O2 has been in for over a 100Kmile and the plugs have been changed it is good practice to replace the O2 as well. Consider it the nth spark plug.

The car has about 220k on the odo and i did change the plugs, wires, cap and rotor. Also changed the oring seal and i need to double check the ignition timing.
I changed the coolant when i got the car about a year ago since i had to replace the radiator. Haven't ever had to add coolant before or after, so I'm going to assume it's not drinking the coolant. I did just replace the exhaust because it had a hole in it and the muffler literally fell off. Not sure if any of that would matter. I'm going to try and test the sensor. Hopefully that goes well.
