Default Idling issues H22a

Have a h22 swap and when I try to start it on first key cycle it does nothing.. Then when I do second key cycle it starts if I give it some throttle.. Id like to get it to start on first crank but idk what to do.

Also when my engine is a operating temp the car will decide to stall out completely as rpms drop all the way to zero while I am in neutral. Sometimes is idles fine in neutral.. It sucks cuz I gotta press brake/ebrake and gas to keep it going when coming to stop.. I took apart FITV and IACV and cleaned both .. Still decides to fall on it's face randomly..

So my three questions basicly are..

Is the engine not getting enough air @ startup to crank over first key cycle?
Why is it idling off and on?
What may cause these things to happen?

you can check the TPS to make sure it's correctly calibrated (see factory manual)

I had idle issues with my obd2 f20b/h23 swaps, idle would jump from 1000 to 3000. Cleaning/adjusting IACV didnt change nothing. I got tired and blocked it off (while still connected) and made my own idle with the throttle stopper screw. The negative side is that i have no idle until my car is warm enough (~100 farenheit), then a shitty idle until it gets to operating temp. Better than nothing is what i branded that fix.
