Icon2 Drive axle loose

Hi there, I have 1994 Honda accord Automatic transmission. My problems is my drive axle on the driver side had been loose for 4th time. Wonder what can be the causes? the first time was due to rear engine mounting crack but the 2nd, 3rd and 4th times dont know what happen. The first one happen sometimes last year while the 2nd and 4th time happened in last two months. Engine mounting seems fine and drive shaft clips also look good. Need you guys expert view. Thanks

Forgot to mentioned that the driver side in malaysia is on the right. The drive axle on the driver side is the shorter one. It was loose when I made a tight right u-turn. Thanks

Is there a snap ring on the end of the shaft that slides into the transmission? There should be.

The snap ring is new when I replaced the new drive shaft last time.

Make sure shaft is fully seated. Proably going to take two people to do this correctly.

This isn't really that hard as long as you do it right. You just need to make sure the snap ring has the opening facing straight down. Then you just jam it in there. If for some strange reason it doesn't, then you can either use a big hammer on the end or you can do it the safe way. They sell a pair of big vice-grips that you can use to snap-lock on somewhere on the shaft. Then use a hammer to hammer the vice-grips to seat the shaft fully into the transmission.
