Could be a lean misfire caused by plugged EGR ports. Or, not.
Need more information about symptom. Throttle position, uphill/downill/flat, RPM range problem is noticed, hot/cold, turning/straight, heavy acceleration, light acceleration, cruising etc.

The car is an M/T. It usually happened when the engine is cold under heavy acceleration in a uphill. Sometime when braking and the Rpm is below 2000, the same problem appears again. The spark plug cable has been changed.
