The leak is going to be in the intake manifold, throttle body area. This is the type of leak that is not going to be visible. Some have used carburator cleaner to spray over the area and if the idle changes that will point to the leak.
It could still be your IACV if it is bad, cleaning it wont solve the idle

i might be picking up a map sensor tonight, and if it changes at all i will post.

fitv, fast idle thermo valve, I would agree, that's what it sounds like, or a vaccuum leak somewhere. Find your jumper plug, if its a 92-93 b18a1, I don't know if the 91 obd0 works the same as obd1 or like the ef obd0 but you should check for that code...
The fitv is directly below the TB will have one coolant line to and one from going to iacv. You need to take this off and take the cover peice with 2 or 3 screws off and adjust the white threaded peice back into its bore and clean as needed.
I googled "fitv" this was third on the list....if it don't work check all the wiring and vaccuum lines for the engine and get that code and go from there.

Just thought of something.... I had this same problem on my zc after I just got it. I had pulled the trans and changed the clutch and when I put it back together I forgot the coolant sensor plug right under distributor side. Without the input to the controler the iacv couldntr regulate the idle, causing these fluctuations...this thows a code.
fast idle thermo valve, for sure man..had it happen to me
its usually on the bottom of throttle body.
pull ur intake (air filtration system) off. and put ur finger over the one of the holes inside the throttle body..one of them should make the car idle down and even out
