Default what could have caused this? damage

The other day I was driving and all of a sudden my shifter popped out of gear (3rd). Had it towed home and pulled the tranny apart and this is the only thing I could find:

its on the 3rd gear on the mainshaft, I lined up the countershaft next to everything and nothing is on the countershaft, no broken teeth or anything like that anywhere. Only symptoms I had been experiencing with the transmission was a knocking sound when the car was sitting at idle in neutral, push the clutch pedal in, knocking goes away. I read up on here a while back this type of knocking usually comes from the mainshaft bearings, about 3 months ago I had the tranny apart for another reason and I did replace the bottom mainshaft bearing (the one that sits inside the housing) and to cover myself now I am going to replace the bearing that sits on top of the mainshaft, but what else could I be looking for that could have caused this type of damage?

Thanks in advance.

Geared was dropped or something in the trans hit it hard or jammed in it. Replace the gear with a new OEM gear or the gear set with a used gear set.

Inspect ALL the teeth on the spider gears inside the diff as well as everything else.

Something was floating around in the tranny to cause that.

the gear was not dropped, when I pulled the case off to inspect, its one of the only things I noticed that was wrong. I ordered the 3rd gear from synchrotech today, just wondering why this happened.

I took the diff out and spun it around, seems normal, I havent found anything that could have been floating around that would have caused this, my thinking is the main shaft was moving around or something.

If the mainshaft moved enough to do that, your trans would be full of metal chunks.

Yeah. Something came into contact with it. I've seen those rings on 3rd spin before (but maybe they do this from factory, I honestly haven't taken too much notice with that). Odd that that was the only sustained damage though. Nothing with the magnet or sitting down with the diff?

nothing abnormal that I noticed, like I said I have double checked every area of the case, everything, nothing floating around that I can see.
