Overheating

Car seems to be overheating. 2 things are happening. It's coming out of the radiator cap and the fan isn't turning on. The cap seems to be securely on and looks to be in good condition. So I guess since the fan isn't cooling it just boils so much it has no where else to go except the cap?
This guy had the fan ghetto rigged. He spliced the wires and had it run directly to the fuse box. No crimping, soldering, just stripped wires tucked into. I took that crap off and had the wires repaired, but it seems the fan doesn't want to kick on. Fuse is good.

well it must be getting extremely hot for the cap to allow it out. when was the last time your thermostat was replaced? and do you have any white smoke from the exhaust ? or a sweet smell coming from the engine?
