if i'm looking at the pics right, it looks like the pic of the head is cyl 3,2,1 and the block is 4,3,2 to clarify for anyone looking at the pics. as riceburner mentioned, looks like you dropped at least one valve and shattered the piston? not sure how you got so much debris in the other cylinders though.
if i had to take a stab since you were wondering if this was tune related, i would say the rev limit was raised, the motor was pushed on a stock valvetrain and you floated a valve. the valve head snapped off, launched itself at the piston, and voila, toasted motor. just a guess, though.

just looking at the pics again though, and they look like aftermarket valves (flat faced and coated). perhaps oversized valves where the bowl was not cut large enough (or at all) and the valves were undercut, reducing their strength. stiff valvetrain slapping the valve against the head (if they weren't seated properly) eventually leading to failure. just another guess. i'm sure there are much more experienced engine builders out there that will know more definitively.

Not my engine, I dont know if the valves are stock or not, valvetrain is ITR, rev limit is 8400rpm.
Stock B16 with ITR cams, springs, rets. AFR tuned to 12.9:1 - 13:1( rally car ) 29* timing from 4000rpm onwards low/hi. No sign of det when dyno tuning using Phormula knock detection and det cans, tuned to MBT. Plugs look good and no signs of det on piston tops.
