Idle Stumbles chuggles and nearly chokes on cold start

Here's the scenario:
It's below 40F. Car it's been sitting overnight.
I get in, turn the key to on, give it a second, then crank it. (Old habit)
It cranks up, fires, and revs to about 1800rpms - 2300rpms.
After it peaks, it gradually slows to about 500rpms and starts to chuggle and shake about 1000rpms before it catches itself and begins to rev back to about 1500-1700 rpms. Here it stays and idles fine until eventually calming to a normal idle of 700rpms. It doesn't happen when it's warm or after the car has been started.
For the record, when it gets really close to 0F, it has chuggled and stalled in the past.
I'm suspicious of the IACV and the FITV as well as a gunky, oily intake. Any suggestions for troubleshooting this problem. I didn't find anything when I searched.
I'm planning to do the whole seafoam through the vacuum line routine in about 1000mi.

The FITV piece was loose, as the FAQ suggested. I spun it down over 4 turns. While I was at it, I cleaned the IACV as well. The screen on the IAC was 100% clogged with oil and other gunk. It took 4 carb cleaner soakings and some gentle toothbrush action to clean the screen. I made an attempt at some before and after shots, shown below.

Overall, I probably spent 1 hour with most of the time going towards fighting with the coolant hoses on the IACV...
Oh, and on a 1990-1993 Accord (F22A Motors), both valves are mounted in easy reach! I didn't even have to remove the FITV, I just had to remove the little plate! How convenient!
